I think you need to upgrade the grant tables:

I did what they say there when I upgraded to MySQL 4.1 and had no problems connecting with Rev.

i'm using:
RunRev 2.0.3
Mac OS X 10.3.8
MySQL 4.1.10-standard

is there a newer version of the library that connects to the MySQL server properly?
