Interestingly enough, the OS X version of this dialog (with buttons "Cancel" and "Replace" does not adhere to the HI guidelines, and makes "Replace" the default...
I know I have seen dialogs/sheets which do the "right thing" and make "Cancel" the default, but now that I am looking for one, I can't seem to find it!
Strange...

This brings up a dialog box which says something like
"File abc.txt already exists. Do you want to overwrite it ?" with choices "Yes" and "No".
The choice to complete the user action would be "yes", but the default would be "no".
(OK my example is actually a Windows one - but the equivalent must exist on Mac).
