Thanks Sarah, Mark & Sims, I just downloaded Iconographer and am giving that a whirl.

I use Iconographer on OS X, which makes the icons for both Mac & Windows. The QTam one made icons in the old format, but Rev 2.something changed to using higher quality Windows icons, so it no longer does the job.
