I use InstallAware http://www.installaware.com/ It's Vista certified and compatible (which many are not) and is very simple to use. It is a tad pricey. However InnoSetup is free (or at least donationware) from http://www.jrsoftware.org/isinfo.php It has some add on tools that make it pretty simple to create an installer. Avoid InstallShield. It's gone from terrible to unbelievably shocking IMHO.
