FWIW, I've never experienced either of these problems (Mac OS 9.1, on PB G3
or PB G4). You might try booting with shift-key held down, which disables
all extensions. Then launch Rev and check dictionary access and quitting.
If they seem much better than you have been experiencing, there's probably
an extension conflict (which I'm sure the Rev team would be keen to learn
about).
