This should be relatively straighforward to do. However I think you
would want to do this for the MainBar rather then for EditBar (the
activation status and trigger is valid for the whole page rather then
for the individual paragraphs)
The MainBar already allows you to add custom buttons so this is the
functionality you would want to explore.

Again same as above - just add button to the main bar and have that
button to open a dialog where you specify name for the page to be
created and upon saving the dialog you create page and redirect to it.
