Oh... and while we're at it, doesn't the formSelection.ftl produce non-valid
HTML for checkboxes?

<label id="${content.controlName}_label"
for="${content.controlName}_${label_index}">
        <input id="${content.controlName}_${label_index}"
name="${content.controlName}" type="${content.type}"
value="${data[1]!data[0]!?html}" ${checked} />
        ${data[0]}
 </label>

If you have multiple select boxes, you'll end up with multiple <label />
elements with the same ID if I'm not mistaken.
