Have you been able to pinpoint in which links that was the case ? - It removes the "html" from the URL in Breadcrumb links but not in the SimpleNavigation links.

It's only an issue if I set the extension to a whitespace empty string " " or try to force a trailing slash "/" on the end. It works ok with nothing in set in the value and any requests to domain.com/url and domain.com/url.html both return an HTML content type page.
