My first guess would be that nodes were not synced before being shut down and upgraded. And then attempt to sync after upgrade resulted in the indexing issue since Lucene was updated as well and indexes have changed. I've looked as syncing of the cluster nodes recently and as far as I could find JR doesn't guarantee when all nodes are in sync and all the entries in journal are written/replayed. The only way to be sure is to wait for while after last update of the node. This would also explain why you ran into issue w/ one system but not w/ another.
