Hi Chris,
there are number of different ways how such changes might be performed. Not 
necessarily via manager. If someone is skilled enough and has enough access 
rights, they can always make changes directly via JCR API. Your best bet in 
logging all such changes is to use observation.

As for GroupManager API being used, can you explain in more details what 
exactly have you been trying to do?
Also please note that if there are multiple managers configured, usually the 
first matching one is used for the job. Is yours configured as first in the 
list?
