I don't have anything technical to add, but I suspect that I have been encountering this for a couple years. The symptom goes back to 2.6.7 (my first 2.6 kernel). However, it has stopped occurring with all but two of my production instances.
Fortunately, this is relatively minor issue that I've designed my systems to work around (kill the guest instance if it hangs at this point).
