I tried a 080414 f265 release with bref=1 bframes=3 and I did not get a
reference B in the mini-GOP.

perhaps you didn't add enough reference frames. You need at least 3 refs
to have a reference B frame. Try with 'ref=3 bframes=3 bref=1'.
Note that adding more than 1 B frame typically degrades quality at the
moment, even with B pyramid. We need the lookahead to determine a good B
frame layout. This is being implemented.
