is a CacheManager active in your testplan?
in this case it is possible (and most liekely intended) that the CacheManager 
suppresses additional sampler calls
(depending from the expires and/or etag header of the last refreshToken 
request/response cycle)

When trying to add the refreshToken in the Test-plan, it gets executed once but

StartScreeningVisit and Debug Sampler gets executed but not the RefreshToken2
