Please excuse if I come in mid-thread and chime in with something totally off but . . .

we had a number of issues with a garbage character appearing on our pages and the <encoding>UTF-8</encoding> wasn't enough.

I remember that we added

<map:serializer
       name="xml"
       src="org.apache.cocoon.serialization.XMLSerializer"
       mime-type="text/html; charset=UTF-8"

in addition to the encoding tag to get rid of the garabage character - note the addition of "charset=UTF-8" in the mime-type.
