Looks like you're using filetemplates on a host with a prefix set.
Filetemplates
will only match against a host without a prefix. If you want to use FTs
in disconnected mode, use php3_midgard_rootfile to specify the location
of the rootfile.
Filetemplates are still a little rough around the edges, though.
