1. Check the database actually exists :) and check that you can log in as
midgard/midgard/midgard
mysql midgard  -umidgard -pmidgard
show tables;

then have a look at
http://www.midgard-project.org/req/midgard-1.4/x242.htm

and see how your apache config differs.
