Not to my knowledge. Although I generally do remove the headers and
libmidgard as a precaution.

You don't need to touch the database. There's a script in the data
package called 'database-upgrade' that will make sure your database
works with 1.4.1.

That's not necesary. If it's a default install, there will be
/usr/local/include/{midgard,mgd_apache}.h, and
/usr/local/lib/libmidgard.*
