I had the same problem when upgrading a 1.4.1 installed from HKLC
scripts from few months back to CVS snap last week, it went away after
uninstalling the older version, making sure all headers were removed as
well, then recompiled the new version and everything started to work.

I'll see if I can find the instructions Emile gave me and mail them to
you.

mod_midgard is working since authentication is handled, also if you
check mysql log you should see that relevant entires are fetched from
db.
