Ok, I think I have found the problem. The first site was using midgard
compiled with sitegroups while the second was using midgard compiled
without sitegroups.

Set loglevel to debug, you'll find a (numeric) reason in the logfile.

Then my guess: this personn 'content' is an SG0 user, and yes, an SG0
user cannot login using the '+' modifier. So I don't know why it's
happening yet, but at least it appears consistent.
