There are two propable causes:
1) there used to be a bug in repligard wich caused sometimes delete
command not to be processed
2) at least in the earlier versions of the approve scripts there was a
logic error that prevented deleted articles from replicating, however
this caused the command to delete the article not to replicate as
well...
As to manually deleting, if you have some kind of admin interface on the
live db you can use that to delete the article manually.
