The first implementation I did used MidCOM for output, but the issue
was with heading levels, as every page started with a <h1> and so
was considered a main chapter.

Performance didn't present issues because we're using caching on the
server anyway. I mean, htmldoc fetching each article in the
documentation tree in "Printer Friendly" mode causes even less
traffic than a search engine spidering the site.

So, maybe now that we can output PDFs the Midgard site could be
qualified as the place for MidCOM documentation? ;-)
