No, the problem here is not the permissions but that Midgard thinks the MidgardPageCacheDir is empty. Otherwise the message here would be "Midgard: regenerating /path/to/cache/dir/4-2-0.php: cannot open, assuming non-existant"

It can't create the cache file because it tries to do so in the root directory (/), which is of course not writable by apache.
