Either use XSSF, or add all your pictures in one go. The way pictures are done in .xls is complicated, and so the current HSSF support zaps old pictures when adding new ones
(We'd love someone to work on fixing this limitation, but it's quite a bit of work and no-one has volunteered to do it in several years... So for now you just need to add all pictures in one go)
