Spark jobs fail with org.apache.spark.shuffle.MetadataFetchFailedException: Missing an output location for shuffle 0

I can run train model successfully, but when I run eval for my model, The error 
is 
org.apache.spark.shuffle.MetadataFetchFailedException:  Missing an output 
location for shuffle 0
