Yeah, I think that happened because there wasn't a proper upgrade
process for people using planet before 3.0 and so your planet instance
has the old urls and the new ones listed in there :/
All you should need to do to fix your problem is delete the old form
urls, the ones that are just /rss/<handle> and leave the ones which are
/<handle>/feed/entries/rss

Actually, I'm not even sure that running the task too often will give
you duplicate entries. The way the entry refreshing works I don't think
that it will duplicate the entries.

That would definitely be a good thing to try. If you remove everything
and refresh and you still have duplicates then you definitely have a
problem with your feed urls.
One other thing to check is to actually look through the list of
subscriptions in the db ...
select * from rag_subscription;
... and see if there are duplicates of any of your feed urls in there.
if a feed url is listed twice, possibly w/ different urls to the same
content, then it could certainly cause duplication.
