After reviewing this more with my colleagues, we are likely to focus the
authentication piece at each application to avoid the Nginx complication for
now.  I would still like an additional central layer on the reverse proxy
server to provide redundant security (in case any app has problems), but
that may be a future project.  I think you are right that will probably
involve writing an Nginx plugin, which is probably outside the scope of this
forum.
