We use Robohelp here: http://www.macromedia.com/software/robohelp/?promoid=BINZ
It can spit out an HTML version of the documentation which even includes Javascript-based searching and index page, and the result is similar to looking at a CHM. Not open-source, and a bit pricey, but gives excellent results (we don't have the latest version, which I presume would be even nicer). -- Frank W.
