Ok; I suggest to double check all paths again, classes, libs, pages... (I
do this every time something goes wrong and I can't find the cause...);
Your code is ok... also you configuration files are ok. I'm 100% sure of
this because I've used your code to make test on my server
instance and everything works fine (I've not reinterpreted your code, I've
done 'copy and paste' from your emails...)
That's why I suggest to check your deployement...
