It seems to me you are doing this wrong :
<sql:substitute-value name="request-param:username"/>
Replace that with
<sql:substitute-value name="username"/>
Look carefully at http://cocoon.apache.org/2.1/userdocs/sql-transformer.html
right bbelow "Substitution".
