IIRC they don't show up under LIST because the specification of LIST is 
braindead, and Abhijit and I were too respectful of the spec.

So how do you access the mail in this mailbox in practice? I 
can't tell my mail clients on any platform to look inside 
/shared for additional mailboxes.

The theory is that clients send the NAMESPACE command, which returns that 
there are shared mailboxes under /, and then clients send something like

a003 LIST "/" "*"

I think shared mailboxes have falled completely out of use as people have 
moved to gmail and other servers that don't support it. What do you want to 
do with them?
