It’s hard to say exactly why this is happening in your specific case. You need 
to check the logs of your management server right at the moment when it occurs. 
Most often it’s because the host investigation gets stuck in a loop of checks 
and can’t determine the status of your node — it can’t confirm whether the node 
has really failed or not.
There can be a huge number of reasons why this happens. As I wrote earlier, I 
never managed to get Host HA working reliably, and in some cases  even 
recommended to disable host ha completely.
If you could give a bit more information — CloudStack version, your OS , and 
most importantly what type of storage you’re using — plus attach the management 
server logs from the exact moment when the HA trigger fires, then maybe I or 
someone else in the community could help pinpoint the issue in your case.
