I am surprised that its hard to get consensus for the idea that the application needs control over which images are cached. I hope that fop will provide some simple control even if the xsl-fo community can't be persuaded of the need.
Yes, that's on the TODO list. It's not quite clear however whether it will arrive as extension attributes or as a hook to plug in a custom Java class. The latter will be implemented in any case.
David Tolpin's idea of relying on HTTP headers to control caching is interesting, but what if you don't use http:/ (e.g. cocoon:/)?
