Aye, it is a pickle. I don't think mod_proexy even sends the remote IP address, it's something that the servlet container handles -- that is, Tomcat sees an incoming connection from 1.2.3.4, and that's it. AFAIK the only way mod_proxy could change that was if it was spoofing it's IP address on the TCPIP level, which is no easy task :)
I almost wonder if this is something that could be added into Cocoon's request.getRemoteAddr() to return the IP given in X-Forwarded-For, otherwise default to the real IP address. This behavior should be configurable, of course.
