I've managed to work around this issue for now by removing any default namespace from my XSL stylesheets, so the resulting XHTML output tree is effectively oblivious of namespaces.
What I am suspecting is that when using default namespaces, somewhere in the toolchain some XML *attributes* (not elements) are considered to be within a namespace when they really aren't. I'll have to look into this a bit deeper. Just in case anyone cares. :-)

This would seem very much to be a bug, and it seems that you are getting close. If you are able to take it to the point at which you can provide a patch for the ExtendedXLinkPipe, I'd be very willing to apply it for you.
