There you find a IMPORTANT pointing to 
http://wiki.cocoondev.org/Wiki.jsp?page=WoodySyntaxUpdateForWidgetsElement, 
where a syntax change is pointed out.

You have to add a wd:widgets element to the form description now.

PS: Is it possible for you to change the real name used at your mail 
account for your mail [EMAIL PROTECTED] into a "real" real name? Otherwise all 
others see the mails as written by "cocoon" as you can read below. Thanks.
