I found the discussion after I sent this question. It seems the result was that the cocoon team decided not to allow redirects from handle-errors.
The link I refered to below is http://cocoon.apache.org/2.1/userdocs/concepts/errorhandling.html in the last text box on the page. The specific issue is that when a user selects a navigation item, instead of showing that item the business tier might throw an exception indicating that the user must see and accept a legal disclosure before proceeding to the requested page. If the disclosure has been previously accepted then the pipeline executes normally. Note that we do not use flowscript and have no plans to (we really like the clear separation of controller from view and model that the sitemap provides). We have actually figured out how to do this by having the action that invokes our business delegate handle the exception and return it in the Map, but this feels akward.
