pipeline.  I can't figure out where in sitemap.xmap or in the XSP page itself 
to ensure that the
output encoding is UTF-8...

this will not be a problem while transforming. your serializer is set to iso-8859-1. Declare your xml serializer like this:

<map:serializer logger="sitemap.serializer.xml" mime-type="text/xml" name="xml" src="org.apache.cocoon.serialization.XMLSerializer">
    <encoding>UTF-8</encoding>
</map:serializer>
