we also had this problems many times.
the solution was to connect to the DB and to modify the flag in table JDOUSER.

I tried a bit further and found out that there are two databases.

I updated the info in the JDOUSERS table, set LOCKED to 'N', changed the 
password, restarted continuum, but still:
