IMPORTANT NOTE: I've also found that this issue only occurs when we have WS-Reliable Messaging enabled for the web service. Also we are deploying on a JBoss EAP 6.3 server but are using the CXF 2.7.6 libraries directly (rather than using JBoss' built-in webservices subsystem).
