I have noticed similar critical problem in OOo forum. And it happened to me again today with OOom151. When editing my odb file I noticed that forms container is empty as if I had no forms which have been just a moment ago.
Looking into odb file by file-roler I found that in content.xml I have no part like this: <db:forms> <db:component db:name="PH01" xlink:href="forms/Obj72" db:as-template="false"/> <db:component db:name="UpravaPodlaISBT2003" xlink:href="forms/Obj91" db:as-template="false"/> <db:component db:name="kontrolne" xlink:href="forms/Obj12" db:as-template="false"/> <db:component db:name="nrt.diagnozy" xlink:href="forms/Obj22" db:as-template="false"/> <db:component db:name="nrt.nambul" xlink:href="forms/Obj32" db:as-template="false"/> <db:component db:name="nrt.nambul2" xlink:href="forms/Obj42" db:as-template="false"/> <db:component db:name="nrt.obce" xlink:href="forms/Obj52" db:as-template="false"/> <db:component db:name="nrt.staty" xlink:href="forms/Obj62" db:as-template="false"/> <db:component db:name="preradenie" xlink:href="forms/Obj82" db:as-template="false"/> </db:forms> I see that forms are in odb file. I can see forms/Objxx directories in odb file. Just this part in content.xml is missing. I cannot reproduce this error but I have suspicion that it has something to do with file recovery after crash. Now it happened after I had to kill soffice because of not responding. After new start and recovery forms have been gone.
