can't say anything here, perhaps some more general list might have

Well, yes, that's a usability issue, IMO. Has nothing to do with Base,
but my very personal opinion - this page does not provide the best user
experience you can ever get in OOo :)
Not sure if it's worth filing an RFE, though.

There has been a bug which prevented correct preservation of the
password, but this should have been fixed in 2.0.4, IIRC.

What do you mean by "restart" - restart of OOo? While the quickstarter
was still running? Or restart of Windows?

Did you get an additional authentication request from OOo?

How exactly did the error message look? Might help to judge whether it's
from OOo (I doubt it) or from the PostgreSQL backend.
