Just tried to reproduce this with the document you attached to your
respective issue (thanks for submitting), with no luck. You might want
to add a more deailed step-by-step description - perhaps there's a
subtle difference in workflow.
Also, please describe how exactly "disappears" manifests - is it
invisible in the IDE, does the respective module vanish from the IDE,
does the IDE close? Any detail, as unimportant as it might seem to you,
might give an idea what's going on.

That holds for the date controls in forms only, not for the date
controls in dialogs (whether or not embedded in forms doens't matter).
Those two are similar and share a common base, but not identical. The
"default-to-today" feature was exclusive to form controls.
