Try delcaring the slider as an object like -
var slider = new GeoExt.ZoomSlider({
renderTo: 'zoom_slider',
height: 100,
vertical: true,
map: map,
plugins: new Ext.ux.SliderTip()
});
And then add the object to your tbar/bbar and see if that works.
