Did you try changing the Icons settings?

I just tried unchecking all the checkboxes and it didn't seem to make a difference. This is with build 3.3.2 OOO330m19 (Build:202) Let me try attaching a screenshot. It shows both problems even though only the header one is relevant.
The list do not allow attachments so you need to use somewhere like Dropbox or Mediafire to place the files then put a link in your message.
