Email list usually are not thread based, unless your client combines
emails with the same subject into thread-like groupings. I think Nabble
does some threading though, but I do not use Nabble often.
Yes, I have seen many lists/forums that are threaded, some good and some
bad.
You are talking about "digest" lists. What are your reasons for using
the digest list? I personally do not use one, so I do not know what the
issues you have.
