I always hint Windows users at a full reinstall every 2-3 yrs. Yeah, it's long, it's boring, but this is the *only* I know of that ensures a 100% clean system.
This way, the filesystem is clean as well. Because of NTFS, I always defrag after installing the system or any piece of software.
