What is the default language for the documents?
Do you have any Language aide extension installed, like Language Tool or
Linguist?
Which version of LO are you using? 3.4.6, 3.5.1, 3.5.2? Windows, Mac,
Linux .deb/.rpm? 32-bit or 64-bit? For Linux - was it download from
LO's site or did it come from a repository or PPA?
That would help us figure out which version you use and look to see if
any one else has the same issue with their copy of that version.
I think I remember seeing that once when I had an early version of
either Language Tool or Linguist installed on my Ubuntu system, but that
was last year some time. So I do not remember what caused it, but I did
see it before.
Give the information I asked you for and maybe someone can replicate the
issue and help fix that issue for you.
Side Question - did you add any dictionary extensions to your default
install, or are you using only the dictionary and such that came with
the default install package [whatever the OS you use]? Some people have
been having issues with the default English dictionaries with version
3.5.x, and it is corrected by installing an add on English dictionary.
I have most every English dictionary I could find listed on my
dictionary page.

Maybe you can add an extension for whatever language[s] you are using
in your document. The link I gave goes directly to the English
section. I have a "kpp" English installed on my system. It seems to
help some issues with the 3.5.2 English dictionary system, according to
some of our user comments in this list. It cannot hurt, but it might
help. If it helps, then that is another item that can be used to
diagnose the problem.
