Hmm, disappointing though. I cannot see how these kinds of things can be avoided if you are editing a manuscript, and I never had such problems in the past with OO in 2002, with a book that certainly went through just as many revisions.
I am clueless anymore how to fix it. If I fix one, another one pops up...
