Hi :) Can you start with a fresh new document created in 4.0.2? Then copy&paste in the contents and formatting from the old document. See if that works. Ideally it would be great to start a fresh document and do all the formatting from scratch to avoid pulling in some old problems but the copy&paste might be enough to leave behind any old unwanted stuff.
I found that with documents that had originally been in MS formats i kept on bumping into weird problems. Then again going from 3.3.x to 3.4.x but since then i've not had any problems.
