LibreOffice 4.0 or LibreOffice 4.1? Works fine for me with 4.0.6 on Debian Wheezy.
Does tables have captions? Check again how Index of tables is inserted, try delete it and insert it again.

I successfully planted such an index after the table of contents, but when I do Tools>Update>All Indexes and Tables the Table of Contents updates with all relevant headings, but nothing happens in the Index of Tables.
